This notice explains what happens to the information you enter in the forms on this website. It covers the Application for Employment, the Wellness Wednesday registration form, and the contact form.

What we collect

Application for Employment. Your name, Social Security number, contact details, education, licenses, work history, references, and your answers to the questions on the form.

Wellness Wednesday registration. Your name, date of birth, age, Social Security number, contact details, employer, physician, the tests you select, and your consent signature.

Contact form. Your name, contact details, and your message.

Why we ask for it

We use the information on the employment application to consider you for employment.

We use the information on the Wellness Wednesday form to register you for the tests you selected.

We use contact form messages to reply to you.

How your information is sent

When you press Submit, your information travels from your browser to this website over an encrypted (HTTPS) connection. The website then sends it by email to the hospital staff responsible for that form.

Email is not a fully secure way to send sensitive information. If you would rather not send your information this way, call us at (402) 684-3366 and we will take it by phone or give you a paper form.

What the website keeps

The website does not keep a copy of what you enter on the employment or wellness forms. It records only that a form was submitted: which form, the date and time, whether the email was sent, and how many fields were filled in.

Messages sent through the contact form are also saved on the website so that staff can refer back to them.

To block automated spam, the website briefly remembers a scrambled version of your internet (IP) address for about 10 minutes. It is not linked to what you submit.

What the hospital keeps

After your information reaches the hospital, it is kept in line with the hospital’s own policies.

Who we share it with

We do not sell your information.

Health information
